The availableplan optionsare theUnitedHealthcare - Standard Plan and the UnitedHealthcare - Premier Plan.They are both Point of Service (POS) Plans. POSis a network of physicians and hospitals that has a contract with the plan to provide medical benefits. Members can choose any provider for care. However, if you choose an in-network provider (i.e. “go in network”), you will receive a higher level of benefits.

Active eligible employees receive medical, prescription, dental and vision insurance within the Archdiocesan health insurance plan. Coverage is available at three different levels – employee, employee + one eligible dependent, or employee + family.

Health Insurance Plan's- Major Provisions and a Comparison Documentfor the plan year 2020- 2021

There are three changes to the Health Insurance Plan effective July 1, 2020:

  1. Vision Benefits: A new vision plan is included with the health insurance plan through DeltaVision. DeltaVision uses the Superior Vision National network, providing one of the larges and diverse networks in the nation.
  2. Dental Benefits: the individual's annual maximum benefit increases from $1,500 to $2,000 and the annual orthodontia increases from $1,500 to $2,000 with the Delta Dental plan.
  3. Health Insurance Rates: The Archdiocese approved an across the board 2% premium increase in health insurance premium rates.

  • A preventative wellness update on theShingles vaccine. Shingrix, a shingles vaccine, is available for approved UHC adult participants over or equal to the age of 50 years old. National convenience care clnics can administer the vaccine some of which are the following: Walmart Care Clinic, Walgreens Healthcare Clinci, MinuteClinic at CVS and Target.
  • AReal Appealweight loss program is available to all UHC participants, spouses and depdnents 18+ with a body mass index of 23 or highter.
  • UnitedHealthcare provides participants aVirtual Doctor Visit. You can access more information and register to receive care by logging into UnitedHealthcare's website athttp://myuhc.com. Click on the "Physicians & Facilites" tab at the top of the page. You can access direct links to provider site where you can register and receive care.

Delta Dental

All Continuation of Coverage/Early Retiree Plan participantsenrolled into one of the two available UHC medical plans are automatically enrolled into the Delta Dental Plan of Missouri. Like the medical plans, you may see any dentist you wish. However, if your dentist participates with Delta Dental your costs may be reduced.Delta Dental of Missouri will mail your new ID card to your home address.

Delta Dental offers two networks, the Delta Dental Premier network and the Delta Dental PPO network. If you choose to see a Delta Dental PPO dentist, you will save money and stretch your benefits. The Delta Dental PPO network gives you 10% more benefits for Basic and Major services. Delta Dental PPO dentists provide greater discounts, saving you even more money. There is no change to your benefits, if you continue to see a Delta Dental Premier dentist or a non-participating dentist.

You and your eligible dependents have the freedom to choose any dentist. Benefits will be based upon your provider and services. You do not enroll in the PPO Network or Premier Network. For example, you may be a patient of a PPO network provider and your dependent may be a patient of a Premier network provider. You are able to change a dentist at any time.
